At St. Charles, our ultimate goal is to prepare you for a return your home or community. We begin these preparations well in advance of your discharge and collaborate with you--the patient--your family/caregivers and your personal team of clinicians to ensure your needs are met. Discharge planning includes the family meeting and falls prevention training.

In addition, patients discharged from our acute rehabilitation hospital programs can choose to continue their rehabilitation through any of our Outpatient Rehabilitation Network locations across Long Island.
